Ongoing Sessions and ADHD Assessments

For all New or Returning Clients: Please complete the appropriate intake form below. Adults should complete the Adult Inquiry Form.

Legal parents/guardians for minors should complete the Minor Child/Dependent Inquiry Form.

If a student has been referred by Jefferson Local School to be seen at the school building, please complete the appropriate form.

If you do not have the ability to compete forms electronically and/or to create a client portal, please call 740-845-8652 to schedule an appointment to come to our Hilliard or London office to complete the forms.

IMPORTANT NOTICES:

After school appointments for children and adolescents are limited and may not be available for new clients.  They are typically offered to existing clients transitioning from daytime appointments.

WE HAVE LIMITED OPENINGS (ONLY AT THE LONDON OFFICE) FOR  CLIENTS WITH MEDICARE PLANS DUE TO EXTREME FEDERAL LIMITS ON WHO CAN PROVIDE SERVICES. We are not able to accept many of the Medicare Advantage plans.

We currently have multiple openings at our Hilliard office and more limited availability at our London office.

For minor clients: All legal documents regarding custody and parenting must be submitted with the inquiry or can be sent to custody@darbycreekcounseling.org and must be received prior to any sessions being scheduled.

Next Steps: After submitting your intake form, we will contact you by email to create a Client Portal. There you will complete additional information, receive an estimated cost for your sessions based on your insurance information and communicate with our staff and your therapist.

If you are referring Darby Creek Counseling, please do not complete the intake form on their behalf. Follow the instructions for professional referrals or direct the individual to complete our intake process on their own.
