The Ohio Administrative Code, Chapter 4757-5, Code of Ethics states: “Counselors, social workers, and MFTs as part of the on-going informed consent process shall obtain any relevant court documents pertaining to custody, visitation, shared parenting, guardianship, or other matters, before proceeding with treatment.”

In accordance with this code, we require all legal documents regarding custody, visitation, shared parenting, and any other such matters, to be on file before any sessions can occur. This ensures that those consent forms for minors are signed by legal custodians and information regarding a minor’s treatment and sessions is only given to those who have legal access to such information.
